Please complete the appointment request form below in full so we have the correct information needed to contact you for scheduling. This information will also help us understand the length of time needed for your appointment and whether your request will require one session or multiple sessions. Once we have come to an agreement on a date and time for your appointment, you will be sent an invoice with a link to pay your deposit. Your deposit is required within 24 hours of invoicing in order to confirm your appointment. Your deposit will go toward the total cost of your tattoo. Please note that deposits are non-refundable for cancelled, or missed appointments.
